The following information is required for the proper filing and recording of Uniform Commercial Code (UCC) transactions:
Financing Statements
All requirements as outlined in the General Document Filing Requirements
Name and address of debtor(s)
Name and address of secured party
Collateral description
Legal description of property if filing in real estate
Debtor's original signature (security agreement okay if it contains the required information and is signed by debtor)
Secured party signatures usually included but not required by law
Prefer UCC-1 form (provided by Secretary of State)
Fees: As per fee schedule.
Continuations
Must identify the original or most current filing by number and date and have original signature of the secured party.
Form must contain name and address of the debtor and secured party as shown on original filing.
Must be filed within six (6) months of the expiration date.
If statement affects real estate, the filing information and legal description must be included.
Fees: As per fee schedule.
Amendments, Releases, Terminations, and Assignments
Must contain both debtor and secured party name and address, the file number being affected and the signature of the secured party.
If affects real estate, filing information and legal description necessary.
UCC-3 forms usually presented.
Assignments also require the full name and address of assignee.

Information/Copy Requests
UCC -11 form usually presented
Name and address of requesting party and name and address of party being searched, as well as any variations of the individual or business name (must be related to the same business)
